Teen Wellness Check-In Questions
Sometimes it can be hard to put your feelings into words. This quick check-in includes 10 simple questions about your mood, stress, relationships, sleep, and daily life.

There are no right or wrong answers. Choose the response that feels most true for you.

It takes about 2–3 minutes to complete.
Our Approach
We use evidence-based therapeutic techniques, including Cognitive Behavioral Therapy (CBT), Dialectical Behavior Therapy (DBT), and Mindfulness practices, to help teens build resilience and coping strategies. Our goal is to create a space where they feel heard, valued, and empowered to express themselves.

What are common signs my teen may be struggling with mental health?

Possible signs include ongoing sadness or irritability, withdrawing from friends or family, changes in sleep or appetite, loss of interest in usual activities, falling behind at school, frequent physical complaints, or avoiding situations they previously handled. A noticeable change in day-to-day functioning is a good reason to check in and consider professional support.

What will my teen actually talk about in therapy?

Your teen does not need to arrive knowing exactly what to say. Sessions may focus on school stress, anxiety, friendships, family conflict, self-confidence, relationships, sadness, or simply feeling stuck. Therapy moves at a pace that feels comfortable, helping teens put words to their experiences and build practical tools for everyday life.

What if my teen does not want to go to therapy?

That is more common than many parents realize. You can let your teen know that therapy is not a punishment and that they will not be forced to share everything immediately. The first appointment can simply be a chance to meet the therapist, ask questions, and see whether it feels like a good fit.
